Responsible for monitoring the quality of life and protecting the rights of residents in long term care facilities. Investigate complaints, conduct routine visits, provide crisis intervention, and work with other agencies. Serve as the lead Ombudsman as required under Volume X of the State of Colorado Human Services Staff Manual.

Knowledge, Skills, & Abilities:
- Receives immediate supervision from the Office on Aging Program Manager.
- May provide consultation and occasional work assignments to the Assistant Long Term Care Ombudsman.
- Exercises supervision over interns and volunteers.
- Consult with the Assistant Long Term Care Ombudsman on complex cases.
- Serve as the lead Ombudsman under the requirements of Volume X of the State Human Services Staff manual.
- Initiate contact with the State of Colorado Ombudsman Program office to consult on issues, policies, and procedures.
- Report to the State of Colorado Ombudsman Program office any service of subpoenas to testify in court, any denial of access to a facility, local complaints against the County Ombudsman program, and any legal action initiated against the local program. Keep State Ombudsman informed regarding any controversial cases involving Ombudsman activities.
- Make recommendations to the Office on Aging Program Manager as to policy and procedure changes and updates for the County Ombudsman program.
- Maintain records and pertinent program data as required by the State Ombudsman and Ombudsman Manager data.
- Prepare written and statistical reports for the Ombudsman program on a monthly and quarterly basis.
- Maintains an ongoing program to recruit, select, and train volunteer ombudsman. Serve as a mentor and supervisor to both volunteers and interns.
- Receive, investigate, and resolve complaints regarding the health, safety, welfare, or rights of residents of long term care facilities in Larimer County.
- Advocate for and protect the rights of residents of assisted living residences and nursing facilities.
- Serve as a liaison between residents, staff, families, and other agencies.
- Conduct routine visits to facilities in order to provide information to residents and to receive complaints, concerns, and requests for assistance.
- Visit and interact with residents, resident representative, and families to explain the ombudsman program and function.
- Develop good working relationships with administrators and staff of long term care facilities. Discuss issues facing them while providing services to aging adults.
- Conduct community education and conduct training to staff in nursing homes and assisted living facilities.
- Identify and work to resolve issues and problems that adversely affect residents.
- Develop and conduct outreach activities to inform the public about the existence of the Ombudsman Program through speaking engagements, on-going involvement with other organizations, hearings, etc.
- Attend seminars, conferences, workshops, classes, lectures, etc., as appropriate, to enhance and maintain knowledge of trends and developments in field; review professional journals, attend association and professional meetings; and otherwise maintain contacts with professionals to facilitate exchange of information.

- At least two years of experience working with aging programs or in a long term care setting or performing in a directly related field or in the performance of similar duties and responsibilities.
- Experience as a Long Term Care ombudsman preferred.
- Equivalent to a Bachelor's Degree from an accredited college or university with major coursework in a Human Services, Social Work, Sociology, Psychology, or a related Human Behavioral Sciences field.
- Current or recent certification through the Colorado State Department of Human Services as an ombudsman preferred.
- If not currently certified as a Colorado Long Term Care ombudsman, must complete required hours and pass certification test within first two months of employment.
- Possession of, or ability to obtain, a valid Colorado Driver's License.
