Provides a wide variety of skilled nursing care and related services to community patients in clinics, homes, or other settings.

  • Receives general supervision from professional-level supervisor or functional manager.
  • Provides direct and indirect supervision of volunteers and students.
  • Administer medications and dosages according to assigned schedules, being aware of potential side effects, and contraindications.
  • Identify health care needs through physical and psychiatric assessments as well as assessments of high-risk patients.
  • Conduct health clinics, screenings and assessments, health promotion, and follow-up activities; perform laboratory work.
  • Prepare patient/client care plans and charts; make follow-up visits to observe patient progress and condition.
  • Discuss patient/client problems and approaches to care with physicians, patient's family, other nursing staff, and outside agencies as appropriate.
  • Gather information from physicians, hospitals, and community resources as appropriate, interfacing with medical professionals as needed, to provide patients with relevant information concerning individual health plans and to ensure appropriate medical care.
  • Maintain accurate and complete patient/client files and records.
  • Serve as a resource to community agencies and medical offices.
  • Recommend changes or revisions to procedures.
  • Attend seminars, conferences, workshops, classes, lectures, etc., as appropriate, to enhance and maintain knowledge of trends and developments in field; review professional journals, attend association and professional meetings; and otherwise maintain contacts with professionals to facilitate exchange of information.
  • Perform special studies and projects as assigned and required.
  • Assist with support services case management as appropriate for assigned programs.
  • Interact with a service network to ensure access to affordable and appropriate services in a timely manner.
  • May participate in various committees and boards related to program activities and responsibilities.
  • May assist with preparation for, and writing of, grant proposals.
  • May work with nursing students to provide community health clinical experience.
  • May participate in emergency preparedness and response activities.
  • Perform other duties as appropriate or necessary for performance of the job.
Knowledge, Skills, & Abilities:
  • General nursing knowledge, principles, practices, objectives, methods, and techniques in assigned area.
  • Medical diagnosis, symptoms, and proper nursing measures in implementing appropriate care.
  • Medical and business software applications including, but not limited to, word processing, spreadsheets, presentation software, databases, and other programs specific to assigned programs.
  • Venipuncture and injection techniques.
  • Protocol for treatment/prevention of communicable diseases and health concerns.
  • Scope and application of laws and regulations pertaining to public health nursing programs.
  • Working knowledge of community resources.
  • Department/division and County rules, regulations, policies, procedures, and standard operation procedures.
  • Correct English usage, spelling, punctuation, and grammar.
  • Perform duties independently without close supervision.
  • Maintain sensitive and confidential information.
  • Follow written and verbal instructions.
  • Perform CPR and respond in a medical emergency.
  • Prepare and maintain clear and concise charts and records.
  • Effectively respond to conflict situations and people in conflict.
  • Communicate clearly and concisely, both orally and in writing.
  • Organize material and present information clearly and concisely, in verbal and written form.
  • Analyze/access information and screenings accurately and take effective action.
  • Train and oversee auxiliary and lay staff providing nursing care in a productive and positive manner.
  • Assist in the process of preparing for, writing, and reviewing grant proposals.
  • Ability to speak and write in Spanish is desirable.
  • Establish and maintain effective working relationships with other County employees, representatives of other agencies and organizations, and members of the community.
  • Up to one year of experience in providing skilled health care.
  • One year of experience in specific public health nursing program.
  • English/Spanish bilingual desired.
  • Equivalent to a Bachelor's Degree in Nursing from a school accredited by CCNE (Commission on Collegiate Nursing Education) or NLNAC (National League for Nursing Accrediting Commission).
  • Licensed to practice as a registered nurse (RN) in the State of Colorado.
  • Possession of, or ability to obtain, CPR and First Aid certifications by date of hire for select programs.
  • Maintain continuing education hours annually, or as required by the Department, and obtain certificates/training necessary for specific programs.
  • Possession of, or ability to obtain, a valid Colorado Driver's License.
