  2. How do I file a complaint or make an inquiry?

    The preferred method to file a complaint, to be sure you provide all of the information we will need, is to download the Department of Human Services Complaint form. This includes if you wish to file an ADA/Section 504 Grievance. You can submit this form electronically or print and mail it to:

    • A brief description of your complaint
    • Your involvement with the complaint
    • What specific outcome you desire
    • The name(s) of the Larimer County Department of Human Services staff person(s) with whom you have discussed your concerns, as well as the outcome of those discussions.

    In general you may obtain a certified copy of a birth certificate by writing or visiting the Bureau of Vital Statistics in the State where you were born.
