Mail-in Ballot Information

Watch County Clerk Scott Doyle's public service announcement video.

All voters in Colorado may cast their ballots by permanent mail-in ballot (formerly called "absentee ballot"). When a voter requests a permanent mail-in ballot, the Elections Department will automatically send a mail-in ballot to the address provided by the voter for all elections conducted by the county. If the voter desires to no longer receive ballots by mail, he/she may contact the Elections Department to cancel the permanent mail-in ballot request. To become a permanent mail-in voter, simply complete and sign the Combination Voter Registration and Mail-In Ballot Application (also available in Spanish). Be sure to check the box next to "Add my name to the list".

Voters may use the Combination Voter Registration and Mail-In Ballot Application (also available in Spanish) to request a mail-in ballot for a specific election if they do not wish to become permanent mail-in voters.

If a voter cannot download the application, a hand-written request is acceptable as long as the following information is provided legibly:

  1. Applicant's full printed name;
  2. Applicant's signature; *
  3. Residence address (and mailing address if applicable);
  4. Address that the ballot should be mailed, if different from residence or mailing;
  5. Date of birth;
  6. Which election they are requesting mail-in ballot for;
  7. Declaration of political party if applicant is requesting Primary Election mail-in Ballot.

* The application shall be personally signed by applicant, or in the case of inability to sign; another person must witness the applicant's mark. Power of Attorney cannot be used to apply for a Mail-in Ballot.

Applications can be either faxed to (970) 498-7845 or mailed to the following address:
